  2. At least 8 high-quality reproductions for each view of the design - 7cm x 5cm.
  3. Name and address of the applicant.
  4. Name and address of the inventor.
  5. Priority document, if priority is claimed.
  6. Power of attorney document.
  7. Information whether or not it is a question of a company design.
