Recording Merger
1.
A certified copy of a merger agreement.
2. A Power of Attorney.
Recording Assignment
1.
Duly executed Deed of Assignment.
2. Power of Attorney.
3. Original registration certificate.
Recordal of Change of Name
1.
An original or a certified copy of document confirming a change of name (this may be an extract from the Commercial Registry, an Affidavit or a Declaration signed by an authorized officer of a company).
2. Power of Attorney.
Recordal of Change of Address
1.
An original or a certified copy of document confirming a change of address (this may be an extract from the Commercial Registry, an Affidavit or a Declaration signed by an authorized officer of a company).
2. Power of Attorney.
Requirements for License Agreement
1.
Three copies of a license agreement (two originals and one notarized copy).
2. Powers of Attorney both from a licensor and from a licensee.
3. A simple copy of a trademark being licensed.
Recordal of Assignment
1. A request for assignment of the application for registration of the trademark duly signed and sealed by both parties.
2. A Power of Attorney in the new owner’s name.
Both documents cited above are not to be notarized and/or legalized.
Also, please note that a notification confirming the recordal of the assignment is usually expected to be issued by the Belarus PTO within one month.
