1. Country in which registration is sought.
2. Full name and address of the applicant.
3. Country and/or state of incorporation, if the applicant is a company.
4. A copy of the mark in the form in which it is to be used.
5. A list of all goods and services in relation to which the applicant is using and/or intends to use the mark.
It is not necessary to file a Power of Attorney for UK applications, but we would require full name and address and (if relevant) company details for our clients and the applicants in such matters.
